I spent nine years in a large hospital system, most of it in general internal medicine. The work I was proudest of happened in the appointments that ran long, and those were the appointments I was told to stop having.
What kept happening was this. Someone would come in exhausted, or gaining weight they could not explain, or cycling through symptoms nobody had connected. Their labs would come back inside range. I would have eleven minutes, and the honest answer was that I had not looked hard enough to say anything useful.
Thrive exists so that looking hard enough is the standard rather than the exception. Ninety minutes for a first visit. A panel wide enough to see patterns. And a review appointment where you see your own results and understand them well enough to explain them to someone else.
